In order to invest with DEC INVESTIMENTOS, we require new members to take the following steps:

1. Fill out a Universal Bank Account Form – Individuals of Banco Itaú S.A. , including the Purchase of Products and Services

• Please print clearly.
• Fill out the name field with your full name, no abbreviations please.
• In the field Products and Services (products and services to be purchased), check NO.
• All fields must be filled out (with the exception of products and services to be purchased).
• For joint accounts, second accountholder’s information must also be provided as well as a copy of his/her Individual Taxpayer Registry (CPF), Identification Card (RG), and proof of residence.

2. Execute the Risk Awareness and Club Adhesion Instrument

• The remaining information will be filled out at a later date by Banco Itaú S.A.

3. Required Documents:
A simple copy of the following documents shall be forwarded to us by the potential investors along with the Universal Bank Account Form and the Risk Awareness and Club Adhesion Instrument:

• Individual Taxpayer Registry (CPF).
• Identification Card (RG).
• Proof of residence under the potential investor’s name (copy must be recent: 30 days or less).

Married women with ID and/or CPF under maiden name must also include copy of their marriage certificate.

For minors, the following documentation must be submitted:

• One copy of minor’s RG. If not available, please include a copy of the birth certificate instead.
• One copy of minor’s CPF, according to the provisions of the Normative Instruction n.190 of the Federal Internal Revenue Department – SRF dated 08/09/2002.
• Copy of guardian’s RG, CPF and proof of residence.
• For joint accounts, please fill out second accountholder’s information must also be provided and his/her documents attached.
